1 Has the participant been ordered to name the alternate payee as irrevocable beneficiary / designated current spouse / designated surviving spouse?
2 Has the participant been ordered to refrain from taking loans and/or distributions?
3 Have unequal death benefits been calculated with have market values been included in a requirement for additional life insurance on the participant?
4 Has the participant been ordered to notify the alternate payee of his/her actual date of retirement?
5 Has the participant been ordered to consent to a release form for verification of accurate disclosure?
6 Have the parties been ordered to execute, without delay, any and all documentation regarding the QDRO order(s)?
7 Do the awarded benefits remain in full force and effect under the terms of the dissolution case, even if the terms of a plan require that any or all benefits revert back to the participant?

10 As the participant been ordered to purchase a life insurance contract equal to the amount of benefits which are not protected from loss in death?
11 Has the participant been ordered to refrain from taking any action (or inaction) that might diminish, delay or extinguish the rights and entitlements of the alternate payee?
12 Has the participant been ordered to pay the alternate payee directly if any awarded benefits are reverted or paid in error?
13 Has the court retained jurisdiction to enter such further orders as are necessary (or to seek other remedy) in the event that the participant or plan administrator fail to comply with the QDRO order(s)?
14 Do the documents contain forms for the parties to stipulate to the order(s)?
